Role: Care Coordinator (Personal Care Aides/CNA Supervisor)
Status: Leadership / Oversight

Location: Centerville, UT


The Role The Care Coordinator serves as the bridge between our operational standards and our front-line care delivery. You will oversee the quality of services provided by our Professional Care Advocates, ensuring that every client receives a "High-Reliability" experience.

Primary Responsibilities:


Professional Oversight: Conduct and document on-site evaluations of Personal Care Aides every six months to ensure compliance with Utah R432-725.

Care Plan Development: Collaborate with the leadership team to design tailored non-medical care plans that reflect the client’s lifestyle and safety needs.


Strategic Referrals: Monitor for shifts in client acuity and facilitate referrals to medical partners if needs exceed the scope of personal care.


Mentorship: Lead and train staff in "High-Reliability" protocols, mandatory reporting, and client rights.


Qualifications & Requirements:
Licensed Nurse (RN/LPN) OR a Certified Nursing Assistant (CNA) with a minimum of two years of verifiable home care experience (R432-725 requirement).
 

  • Profound understanding of Utah Administrative Rule R432-725.

  • Experience in a supervisory role within a high-standard or boutique environment.

  • Must be at least 21 years of age.

    System Integrity: The employee shall only access, use, or disclose the minimum necessary information required to perform their job functions. All data must be processed through the company’s designated secure external portal. Unauthorized sharing, downloading, or local storage of client information is strictly prohibited and constitutes grounds for immediate termination.

Personal Care Aide

Status: Part-Time / Full-Time / PRN

Location: Centerville & Northern Utah
 

The Role As a Professional Care Advocate at Utah At Home Care, LLC, you are a guardian of dignity and independence. You will provide elite, non-medical support to clients in their private residences, operating within a high-reliability framework that prioritizes safety and professional precision. This is not just caregiving; it is an exercise in operational excellence.


Primary Responsibilities: Activities of Daily Living (ADLs): Provide meticulous assistance with bathing, dressing, grooming, and personal hygiene, ensuring the client’s comfort and dignity at all times.
 

  • Wellness Monitoring: Observe and document daily health and safety indicators, identifying subtle changes that may require a professional referral.

  • Companion Excellence: Engage in meaningful social interaction, cognitively stimulating activities, and thoughtful companionship.

  • Household Management: Perform light housekeeping, meal preparation, and laundry services to maintain a serene and high-standard home environment.

  • Operational Discipline: Adhere to all safety protocols and document all services provided within our secure digital client portal.
     

Qualifications & Requirements * Must be at least 18 years of age (R432-725 requirement).
 

  • Current CPR and First Aid certification.

  • Demonstrated competency in personal care standards and non-medical safety protocols.

  • Exceptional communication skills and a refined, professional demeanor.

  • Ability to pass a comprehensive criminal background screening.
