1. Information We Collect Utah At Home Care, LLC ("we," "us," or "our") only collects personal information that you voluntarily provide through our website contact forms. This includes:

  • Email Address

  • Phone Number

  • Type of inquiry (Self or Loved One)
     

2. How Information is Stored No sensitive client health information or protected health information (PHI) is stored on this website. All formal client onboarding, care plans, and health-related communications are conducted through our secure external HIPAA-secure client portal. Our website serves only as a point of initial contact.
 

3. Use of Information We use the contact information provided on this site solely to:

  • Respond to your initial inquiries.

  • Schedule your free comprehensive assessment.

  • Provide information about our home care services.
     

4. Third-Party Sharing We do not sell, trade, or otherwise transfer your personally identifiable information to outside parties. This does not include trusted third parties who assist us in operating our secure portal or conducting our business, so long as those parties agree to keep this information confidential.

Client Rights & Records (R432-725-13)

Client records are maintained for a minimum of three years following the last date of service. All records are stored securely via a HIPAA-compliant external portal to protect against unauthorized access.

Mandatory Reporting Disclosure

In Utah, anyone who has reason to believe that a vulnerable adult has been the subject of abuse, neglect, or exploitation is a Mandatory Reporter under Utah Code § 76-5-111.1. We reserve the right to disclose personal information as required by Utah law, specifically in compliance with mandatory reporting requirements for suspected abuse, neglect, or exploitation of vulnerable adults.
